National Youth in Transition Database (NYTD) Survey
We want
to know what you think.
We want all youth to be successful and this is your chance to help. As of October 2010,
the state of Hawaii has been collecting information to help ALL youth in foster care across the nation. Your feedback,
along with other foster youths from other states are put into a database to help improve
services provided especially for you and those who come after you. This survey is taken soon after you turn the age of 17, again
at age 19, and finally at age 21.
The questions on this survey should be answered from YOUR perspective.
Don’t get discouraged if you don’t know some of the answers – this is not a test and you will not be
graded. Your fair but honest input will help us to evaluate the
effectiveness and the needed changes of our work.
What YOU say really matters. Remember WE CAN’T DO THIS WITHOUT YOU!!
